On the word you'd like to add to your custom dictionary, do a right-click or Control+click, select Spelling, and in the Spelling and Grammar dialog click Options. The Spelling and Grammar preferences pane should be displayed. For Custom Dictionaries, click on the Dictionaries button, put a check mark in the Custom Dictionary item box, and OK. But theres no obvious way to remove a word from the dictionary if you accidentally add one.
